Please use this form to notify the circulation department that you have lost a book charged to your ID. Once you have declared a book lost, overdue fines cease to accrue as of the date it is reported lost. Our staff will research the replacement cost of the item and bill you accordingly.
Should you later locate and return the item declared lost, appropriate monies will be credited to your account. All boxes on the form with RED labels are required information, and must be completed in order to properly expedite your request.
